PHYSICALS AND IMMUNIZATIONS 

Each student of the Merrimack School District is required to have updated immunizations and a physical completed within the last two years. If you are a NEW student to our district and are coming from another state, you are required to have a physical on file from within the last one year.   Please make sure that you are providing the Nurse with a copy of any updated forms you receive when bringing your child for their exams/immunizations.  If your child has a medical or religious exemption from immunizations, please complete the exemption form (link below) and submit it to the Nurse at the beginning of the year.

All students in NH are required to be up to date with their TDAP vaccinations prior to the start of their 7th grade year. Please make sure you are submitting these records right away in the fall or over the summer.  You can email/fax/mail or drop off forms to the office at MMS.
